    After their sequestering on reservations across the West, American Indians suffered from appalling rates of disease and morbidity. While the United States Indian Service (Bureau of Indian Affairs) provided some services prior to 1908, it was not until then that the Indian Medical Service was established for the purpose of providing services to American Indians. Born in an era of assimilation and myths of vanishing Indians, the Indian Medical Service provided emergency and curative care with little forethought of preventive medicine. DeJong argues that the U.S. Congress provided little more than basic, curative treatment, and that this Congressional parsimony is reflected in the services (or lack thereof) provided by the Indian Medical Service. DeJong considers the mediocre results of the Indian Medical Service from a cultural perspective. He argues that, rather than considering a social conservation model of medicine, the Indian Service focused on curative medicine from a strictly Western perspective. This failure to appreciate the unique American Indian cultural norms and values associated with health and well-being led to a resistance from American Indians which seemingly justified parsimonious Congressional appropriations and initiated a cycle of benign neglect. 'If You Knew the Conditions' examines the impact of the long-standing Congressional mandate of cultural assimilation, combined with the Congressional desire to abolish the Indian Service, on the degree and extent of disease in Indian Country..

    Licensed to Practice begins with an 1891 shooting in Wheeling, West Virginia, that left one doctor dead and another on trial for his life. Formerly close friends, the doctors had fallen out over the issue of medical licensing. Historian James C. Mohr calls the murder "a sorry personal consequence of the far larger and historically significant battle among West Virginia’s physicians over the future of their profession."

    Through most of the nineteenth century, anyone could call themselves a doctor and could practice medicine on whatever basis they wished. But an 1889 U.S. Supreme Court case, Dent v. West Virginia, effectively transformed medical practice from an unregulated occupation to a legally recognized profession. The political and legal battles that led up to the decision were unusually bitter―especially among physicians themselves―and the outcome was far from a foregone conclusion.

    So-called Regular physicians wanted to impose their own standards on the wide-open medical marketplace in which they and such non-Regulars as Thomsonians, Botanics, Hydropaths, Homeopaths, and Eclectics competed. The Regulars achieved their goal by persuading the state legislature to make it a crime for anyone to practice without a license from the Board of Health, which they controlled. When the high court approved that arrangement―despite constitutional challenges―the licensing precedents established in West Virginia became the bedrock on which the modern American medical structure was built. And those precedents would have profound implications. Thus does Dent, a little-known Supreme Court case, influence how Americans receive health care more than a hundred years after the fact.

    Tony Kushner’s plays include Angels in America; Hydriotaphia, or the Death of Dr. Brown; The Illusion, adapted from the play by Pierre Cornelle; Slavs!; A Bright Room Called Day; Homebody/Kabul; Caroline, or Change, a musical with composer Jeanine Tesori; and The Intelligent Homosexual’s Guide to Capitalism and Socialism with a Key to the Scriptures. He wrote the screenplays for Mike Nichols’s film of Angels in America and for Steven Spielberg’s Munich and Lincoln. His books include The Art of Maurice Sendak: 1980 to the Present; Brundibar, with illustrations by Maurice Sendak; and Wrestling with Zion: Progressive Jewish-American Responses to the Israeli-Palestinian Conflict, co-edited with Alisa Solomon.
    Among many honors, Kushner is the recipient of a Pulitzer Prize, two Tony Awards, three Obie Awards, two Evening Standard Awards, an Olivier Award, an Emmy Award, two Oscar nominations, and the Steinberg Distinguished Playwright Award.     Tony Kushner's Angels in America is that rare entity: a work for the stage that is profoundly moving yet very funny, highly theatrical yet steeped in traditional literary values, and most of all deeply American in its attitudes and political concerns. In two full-length plays--Millennium Approaches and Perestroika--Kushner tells the story of a handful of people trying to make sense of the world. Prior is a man living with AIDS whose lover Louis has left him and become involved with Joe, an ex-Mormon and political conservative whose wife, Harper, is slowly having a nervous breakdown. These stories are contrasted with that of Roy Cohn (a fictional re-creation of the infamous American conservative ideologue who died of AIDS in 1986) and his attempts to remain in the closet while trying to find some sort of personal salvation in his beliefs.

    But such a summary does not do justice to Kushner's grand plan, which mixes magical realism with political speeches, high comedy with painful tragedy, and stitches it all together with a daring sense of irony and a moral vision that demands respect and attention. On one level, the play is an indictment of the government led by Ronald Reagan, from the blatant disregard for the AIDS crisis to the flagrant political corruption. But beneath the acute sense of political and moral outrage lies a meditation on what it means to live and die--of AIDS, or anything else--in a society that cares less and less about human life and basic decency. The play's breadth and internal drive is matched by its beautiful writing and unbridled compassion.
